UKVI does not require a booked flight. A planned itinerary set out in your cover letter is enough, and paying for tickets before a decision is simply money at risk.
Where people slip: Buying tickets to look serious. It does not help the application, and if the visa is refused the money is gone.
How to hold a flight without payingHotel booking
Show where you will stay — a booking, or an invitation letter carrying the host's address and their immigration status in the UK. It does not need to be prepaid.
Where people slip: Staying with family and submitting nothing about the host. The officer needs their name, address, status and relationship to you.

Not a UK visa requirement. The Immigration Health Surcharge applies only to stays over six months; visitors do not pay it.
