The Czech Republic Medical Treatment Visa is a Schengen Short-Stay (Type C) visa for Indians travelling for specific medical treatment in the Czech Republic. Requires a doctor's certificate confirming need for treatment, an official letter from the receiving Czech medical institution confirming acceptance, and proof of pre-payment of treatment. Stay up to 90 days. Travel companions (one or two close family members) can apply on the same purpose with proof of relationship.

Get the Indian doctor's recommendation letter
On hospital / clinic letterhead — confirming the need for specific treatment and why it's being sought abroad. Translate to English if originally in another language.
Secure the Czech medical institution acceptance letter
On the Czech hospital's letterhead — confirming they can perform the specific treatment and that you've been accepted. Include treatment plan + estimated duration.
Pre-pay the treatment (or document the sponsorship)
Bank wire transfer / hospital invoice paid. If sponsored by an organisation or family, include sponsorship letter + sponsor's bank statements.
